CCHP has developed a pediatric obesity disease management program to help develop resources and track obese children aged 2 through 11. Please refer us your eligible children with BMI% for age 95% or higher. We will contact the families and send them information about healthy lifestyles quarterly. We will also give you feedback and assist with referrals every 3-6 months.
The referral is easy; just type Pediatic Obesity into the Medication/orders part of cclink. The CCHP Pediatric Obesity program will come up. Please complete the referral which verifies the eligibility of the child/family and asks a few questions. We will confirm the referral once we send the family information and put them into our registry.
